Mark Toles, PhD, RN

Mark TolesMark Toles, PhD, RN is an assistant professor with clinical experience in transitional care, behavioral health and nursing management. In his research, Dr. Toles seeks to expand understanding of transitional care for older adults and their family caregivers. In epidemiological and observational studies, he described the prevalence of re-hospitalizations for older adults after nursing home discharge and targets for improving the ability of nursing home staff to deliver transitional care and improve patient outcomes. Dr. Toles is developing an intervention to improve transitions in care for older adults and family caregivers who transition from skilled nursing facilities to home. He is a former John A. Hartford Foundation Predoctoral Scholar and Claire M. Fagin Postdoctoral Fellow. His research is currently supported by the National Center for Advancing Translational Sciences, National Institutes of Health, through Grant 1KL2TR001109.
