Merck Program for Assistance with Transitions from Hospital to Home (PATHH)

The purpose of the study is to determine impact of the Hospital Transition in Care (HTiC) Service on the following patient outcomes: 1) 30-day hospital readmission rate 2) Emergency room visits within 30-days after discharge 3) Patient satisfaction 4) Patient knowledge of discharge instructions 5) Patient adherence with discharge plans, In addition, the project seeks to test the technology and integration required to successfully execute, operate, and scale the HTiC Service and to identify opportunities to improve its design.
